Wellbeing 4U

Important information regarding service amendments for Wellbeing4U – we are still here to help you support your patients

  • 121 sessions – During this difficult time, we are continuing to support your patients through telephone appointments. We are aligning our service at present to support patients by giving them the opportunity to talk through Public Health advice and guidance surrounding Coronavirus, make plans for keeping themselves well whilst being isolated at home, along with making links to relevant community assets available at this difficult time or access regular check in calls. The online referral has an option for Coronavirus support and guidance.
  • Healthful Network online courses – We are aligning our Healthful Network offerings to provide online courses. The timetable and details of these courses will be circulated in the next few days.

Community Nursing

With the current situation, we are diverting all nursing resources from any routine, non-essential services into our core community nursing services to support District Nursing and ART.

Home visits will be reduced as much as possible, to create capacity to prioritise nursing care for end of life and palliative patients in the community.

During this time we will be unable to do routine bloods at home.

We will continue to accept referrals for essential INR’s for housebound patients and urgent bloods for acutely unwell patients.

From Friday 20th March the Community Tissue Viability nursing service will be temporarily closed and will not be accepting any routine referrals.

From Friday 20th March, the Community Continence service will be temporarily closed and will not be accepting any routine referrals.

The only exception is referrals for palliative and end of life patients who require products urgently and these referrals will be processed in the usual way.
